Medicare B clients must present their card at the time of service.

Individuals with Medicare Managed Care (Humana, Sterling Life) will have to pay $25.00
bullet

An invoice will be made available to send to your managed care for reimbursement

bullet

Medicaid Managed Care (Molina, CareSource, AmeriGroup and Buckeye Community Health Plan) clients must present card at time of service.

People who should get vaccinated each year are:

bullet

Children aged 6 months up to their 19th birthday

bullet

Pregnant woman

bullet

People 50 years of age and older

bullet

People of any age with certain chronic medical conditions

bullet

People who live in nursing homes and other long-term care facilities

bullet

People who live with or care for those at high risk for complications from the flu, including:
bullet

Health care workers

bullet

Household contacts of persons at high risk for complications from the flu

bullet

Household contacts and out of home caregivers of children less than 6 months of age (these children are too young to be vaccinated).

ELIGIBILITY FOR PNEUMONIA VACCINATION

65 years of age or older
bullet

Those that are younger will need to bring a prescription from your physician

bullet

Anyone over 2 years of age who has a long term health problem

bullet

Anyone over 2 years of age who has a disease or condition that lowers that body's resistance to infection
