bullet New Food Service Requirements - A detailed floor plan, a complete menu, equipment model numbers and NSF/UL numbers, and a plan review fee  must be included with the application. 
 
bulletFood Service Application - To apply for or renew your food service license, print out the application, fill out the applicable information and submit appropriate fee. Contact Clinton County Health Department at 937-382-7251 for current license fees.
 
bulletTemporary Food Service - A temporary food service is a food service operation that is operated at an event for not more than five (5) consecutive days, except when operated for more than five consecutive days pursuant to division (E)(2) of section 3717.43 of the Revised Code.
