*** As of October 3, 2005 - Price of Certified Birth and Death Certificates Have Increased ***

As of October 3, 2005 the cost of a Certified Birth or Death Certificate will be $17.00.  $3.00 of this goes to the Children's Trust Fund, $7.00 stays with the Health Department and $7.00 goes to the State of Ohio for improvements in their computer programs in Vital Statistics.

The State has also notified the Health Departments that Uncertified Copies and Wallet size Birth Certificates will no longer be issued beginning July 1, 2003. 

bulletPublic Records are available daily from 8:00 a.m. - 4:00 p.m.
 
bulletBirth and Death Certificates (Certified or copies) are available from the years 1908 to the present day.
bulletFor newborn babies, there is a 4 to 6 week delay before records become available.
bulletOnly persons who were born or who died in Clinton County are available through this office.
bulletCertified copies (certified with the raised State of Ohio seal and registrar's stamp) of Birth and Death Certificates are $17.00.
 
bulletCertified copies of Birth or Death Certificates may be obtained by mail or in person.

bulletTo request a copy by mail, submit the name and birthdate (or date of death) of the person. Send a Money Order for the correct amount and a Self Addressed Stamped Envelope to:

bulletTo request a copy in person, the office is located at the address shown above and the hours are 8:00 a.m. - 4:00 p.m. daily. You will be asked to fill out a request form for our records. The only information required is the person's name and birthdate ( or date of death).

bulletCurrently, we only accept personal checks within Clinton County ( payable to Clinton County Health Department ) and cash ( or money order).
